An employee was operating an unloaded forklift on a paved surface outside of the building to return the forklift to the parking lot. When the forklift turned, it tipped over and the employee, who was wearing a seatbelt, remained in the seat with one leg turned up under him and the other behind him as he faced the ground. His left foot and right leg were fractured.

An employee was checking a top seal machine to determine why the bags were not heat sealing properly when he contacted an energized 240 volt, 20 amp electrical wire and received electrical burns to his right hand.

An employee was working at the log deck of the hew-saw. He was attempting to free a stuck log when the rotary feeder pushed the log, causing his left foot to become pinned between the log and the wall of the equipment. This resulted in a fracture to his left foot, requiring hospitalization. The rotary feeder was in operation at the time of the incident.

An employee was checking for proper alignment of the Hewsaw Machine. He placed his left hand by the saw box thereby crushing it and resulting in the amputation of his pointer and pinky fingers. He was taken to the hospital where he remained overnight for surgery to attach the fingers.
